Online enrolment form

The first time you make a booking, you'll need to complete the online enrolment form. You'll also be asked to confirm the details are correct when you make subsequent bookings.

There are eight sections to complete on the form shown below.

Online enrolment form
  • Parent / Primary Guardian Details
  • Additional Questions
  • Parent or Guardian #2
  • Emergency Contacts
  • Child's Details
  • Child's Health Details
  • Additional Questions for Child
  • Final Step
Some questions are optional — only answer them if relevant. Others are mandatory, and Enrolmy won't let you save the form until they're completed.

The parent/primary guardian should be the person who mainly makes bookings. If more than one person may make bookings, a parent/guardian #2 can be added. The child details are for the child who will attend the sessions. It's important to keep the information up to date, particularly contact and health details.

The final step is to click the 'I'm finished' button to submit the form.
